Please provide us with the name and birthday of each of your children who are under the age of 18. This includes grandchildren who are in your care or who attend with you. If they have a different home address, please include that in the blank space below. We will not text or email children under the age of 18 unless you as their parent or guardian give us permission. This is something that must be done in person.
